KNEE TREATMENT
A Game Plan Built Around Your Goals
An injured or painful knee doesn’t automatically mean surgery. Dr. Maguire takes a conservative approach to knee care and carefully considers your diagnosis, symptoms, activity level, and expectations before recommending treatment.
For many patients, non-surgical solutions can relieve pain, improve strength and mobility, and provide a safe path back to sports or daily activities. When surgery is the better option, Dr. Maguire will explain why, answer your questions, and help you understand what to expect from treatment and recovery.
Depending on your condition, your individualized treatment plan may include:
Our Procedures
Advanced Knee Surgery Options
When conservative treatment isn’t enough, or the nature of an injury makes surgery necessary, Dr. Maguire uses modern surgical techniques to repair damage, preserve healthy tissue, and minimize downtime.
Arthroscopic Knee Surgery
Using a tiny camera and specialized instruments inserted through small incisions, Dr. Maguire can treat many knee injuries with minimal disruption to surrounding tissue. Knee arthroscopy can be used for procedures such as meniscus repair or debridement, cartilage treatment, and certain ligament procedures. Compared to open surgery, arthroscopic techniques may result in smaller scars, less post-operative pain, and a faster recovery.
ACL/PCL/MCL/LCL Repair & Reconstruction
Dr. Maguire treats isolated and complex knee ligament injuries with procedures including PCL reconstruction, MCL, LCL, and ACL repair and reconstruction, and multi-ligament knee reconstruction. The goal is to restore stability and help patients get back to sports, exercise, and the activities they love.
Meniscus Surgery
Depending on the type, location, and severity of a meniscus tear, Dr. Maguire may perform meniscus repair surgery to stitch the cartilage back together or debridement to remove the damaged tissue that’s causing pain, catching, or other symptoms.
Cartilage Restoration & Joint Preservation
For certain patients with damaged knee cartilage, advanced restoration techniques can help preserve the natural joint and delay or avoid knee replacement surgery. Dr. Maguire performs procedures including cartilage restoration, autologous chondrocyte implantation, and osteochondral allograft transplantation.
Kneecap & Extensor Mechanism Surgery
Dr. Maguire treats kneecap instability and injuries that affect the structures that allow the knee to straighten. Procedures include dislocating kneecap repair and reconstruction, patellar tendon repair, quadriceps tendon repair, tibial tubercle osteotomy, anterior tibial tubercleplasty, and lateral retinacular release.
Knee Osteotomy (Realignment) Surgery
If knee malalignment contributes to pain, instability, or uneven joint loading, Dr. Maguire may recommend an osteotomy to redistribute pressure across the knee and preserve the natural joint. Procedures include high tibial osteotomy and distal femoral osteotomy.
Partial & Total Knee Replacement
When advanced knee arthritis causes persistent pain and loss of function that no longer responds to conservative treatment, joint replacement could be the most effective option. As a knee replacement surgeon on Long Island, Dr. Maguire performs both partial and total knee replacement. He selects the best approach based on the location and extent of joint damage.

FAQs
Can an ACL tear heal without surgery?
While a ligament strain might heal on its own with proper care (i.e., rest, physical therapy), unfortunately, a partial or complete ACL tear won’t fully repair itself because the ligament has poor blood supply.
Factors such as the severity of the tear, your age, activity level, and the sports or activities you hope to return to will influence whether conservative treatment or surgery is recommended. Dr. Maguire will evaluate your individual injury and discuss all of your options so you can make an informed decision.
Does a torn meniscus always require surgery?
No. Some meniscus tears can be managed with rest, physical therapy, medications, or other conservative treatments. In the case of small tears in areas with adequate blood supply, the meniscus could heal on its own. ACL tears, however, may not heal without surgery due to poor blood supply and because the ligament has been disrupted and no longer has contact with its insertion site or remaining ACL fibres.
Surgery might be considered if knee pain and swelling persist, the knee locks or catches, or mobility is significantly reduced.
What's the difference between ACL repair and ACL reconstruction?
ACL repair involves reattaching the torn ligament to the bone. ACL reconstruction, on the other hand, involves placing a graft to effectively create a new ligament.
The appropriate technique depends on the location and severity of the ACL tear.
How long does it take to return to sports after knee surgery?
Return-to-sport timing varies by patient and procedure. Dr. Maguire will provide you with a timeline based on how quickly you heal and the demands of the sport you play.
When should I see a knee doctor on Long Island?
You should schedule an evaluation with a knee doctor on Long Island if you have:
- persistent knee pain;
- instability;
- decreased sensation;
- ongoing swelling;
- limited movement;
- pain that interferes with sleep, sports, or daily activities;
- visible deformity of the joint;
- locking or catching;
- trouble walking; and/or
- difficulty putting weight on the injured leg.
You should also see an orthopedic knee surgeon if you’re an athlete who sustained a traumatic knee injury (i.e., a hard fall, collision, twisting injury). When it comes to sports injuries, early evaluation can often produce the best outcome.
When is knee replacement surgery necessary?
Knee replacement is generally considered for patients with advanced arthritis or significant joint damage when pain and loss of function continue, and conservative treatments no longer provide relief.
As a knee replacement surgeon on Long Island, Dr. Maguire performs both partial and total knee replacement. He’ll evaluate the extent of your condition, explain your options, and collaborate with you to decide if, and when, knee replacement surgery is the best course of action.
Do you treat athletes with knee injuries?
Yes. As a fellowship-trained sports medicine orthopedic surgeon, Dr. Maguire treats athletes at all levels with injuries such as ACL, PCL, LCL, and MCL tears, meniscus tears, cartilage injuries, and ligament sprains.
During his fellowship at MedStar Georgetown University, Dr. Maguire participated in medical coverage for professional and collegiate sports teams, which exposed him to a range of sports injuries.
Do you treat non-athletes with knee pain?
Absolutely. Dr. Maguire treats patients of all ages and backgrounds, including working professionals, active adults, and older adults dealing with knee arthritis, injuries, and wear and tear.
